ESA Work Capability Questionnaire was created by Mikey
This question is related to completing the form on a computer and then printing out the document.

If a claimant only suffers with mental health problems and the 'Physical Functions' part of the questionnaire hasn't been completed would they still need to print out the 'Physical Functions' part and send it with the rest of the document?

Also; should the phone number of the claimant be included?
What if a claimant - due to mental health issues - only answers her phone to recognised numbers such as her doctor and carers?

I'm afraid that they need to print out the whole form or they risk it being rejected as incomplete as an alternative you could provide your answer on separate sheets and return the original form.

It is perfectly OK for you to type your answers and attach them as extra pages to the ESA50, just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.

Attach the pages to the back of the ESA50 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

You do not need to provide a phone number and if you do not want to be contacted by phone then include a note about this with the form.

With regarding to providing a phone number.
At the stage of a mandatory reconsideration I think I read on here that a claimant will be contacted 3 times by phone to inform her if the claim is going to the appeal stage. If this is correct how would a claimant be contacted if a phone number isn't provided due to a claimant only answering her phone to recognised numbers such as her doctor and carers?

There is no requirement for the DWP to contact the claimant in these circumstances and they don't in the majority of cases, however, if you want them to have the opportunity to call you then you will have to consider providing them with a phone number.

All Decisions have to be notified to the claimant in writing.
