PIP CLAIMANT REQUEST for GP COMMENT was created by Sooty2
This is my first post. Thank you for all the helpful advice and guides. I am moving from indefinite Higher Rate Mobility to PIP. I would appreciate your help. My GP has kindly completed your downloadable GP comments tick sheet and made comments. I wondered if I should specifically refer to it in the various sections of the PIP form? She has made some comments about aids and adaptations in my home. She has also kindly written a letter. Many thanks.

Thank you Gordon. I will check through the PIP Guide again. I must have missed the section about how to present the GP comment form, with my claim. Many thanks.

Forgive me I've only had a couple of hours sleep and thought I had read your post fully, but obviously did not.

Yes you need to refer back to the GP's comments in your answers to each question, you should do this with whatever evidence you have available wherever possible. You are trying to show that this is not just your opinion over the limitations that you have.
