Got sent my report today and it is riddled with lies .they have even changed my sex to female in some parts it says she had to use her inhaler at the assesment they have written a whole page of exercises that was not done at all.the nurse told me there was no need to do any not even the walking test she even had to get me a wheelchair to use to go to toilet yet the report says I was observed walking 30yards .the report says I only need 1 knee replaced yet my letter which I had given her from my doctor says I need 2.i have now asked for a reconsideration

I would certainly mention the problems with the report but be aware that if it is found to be unfit for purpose then it can result in a new assessment being required.
That said, don't lose track of the fact that the Decision will only be revised by your showing that you meet the criteria to score the extra points that you need for an Enhanced award.

Phoned them and pointed out the mistakes she couldn't understand why it says "she had to use her inhaler" and why it says I only needed 1knee replacement when it should have said 2 then she she said it would defo need a reconsideration but they texted me today and said they will write to me within 6weeks of their decision.

It sounds as if they have registered an MR against the Decision, if you are intending to provide more information then I would do so as soon as you can.
