Hello, I have been invited to apply for PIP from DLA. I receive the lower rates of DLA for both mobility and care. My needs are increased since I first applied for DLA, and I never had a face-to-face assessment in all of the years I claimed. I'm petrified about this next step, to say the least! I called DWP to tell them I wished to claim PIP, before I had found this site, they only questions they asked were about my GP etc, so not sure if I did that correctly. I have downloaded the PIP claim guide, completed a rough draft of the claim form, but would welcome any assistance. This is extremely stressful, I am aware I will more than likely have to attend a face-to-face, though obviously I would want to avoid this if possible. I have little in the way of evidence, mental health problems would need my GP to confirm 9though I may have historic CPN care plans, I don't have a physio care care plan, I have previously attended a Pain Clinic. Diabetes, again, I would need evidence from my GP, Fibroymlgyia again from my GP, arthritis physio, thyroid, high blood pressure etc again my GP. I can send a copy of my repeat prescription list. with regards of the sections that you can enter extra informatio, can these be typed up and attached to the claim form? Can anybody help, please

I would certainly speak to your GP about any support they can give you.

Have a good read of the sections on fluctuating conditions and the use of Relibly.

There's a formula in the guide to help you answer the questions.

Treat the Descriptors as they are written, don't be tempted to re-interpret them to fit better to your problems.

Thank you for replying.

Can the sections that require 'Extra Information' be typed up?

I am struggling to find the formula in the guide, perhaps I am understanding what you mean by formula, apologies.

Do I need to answer the questions with the word 'reliably' included, or just answer if I can or cannot do whatever the questions are asking 'reliably'?

Sorry for all the questions. I have numerous conditions, and whilst some of them can be described and relate to the questions asked, some of them like high blood pressure are just 'run of the mill' illnesses, but I assume I should list every one at the beginning of the form?

It is perfectly OK for you to type your answers and attach them as extra pages to the PIP2, just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.

Attach the pages to the back of the PIP2 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

The "formula" follows the same basic structure and is at the end of each Activity section with some examples based on a variety of conditions.

1. What is the health condition and what are the symptoms that cause you problems with the PIP activity? For example, stiffness caused by arthritis or fatigue caused by ME.
2. Describe the problems you have, giving details and examples if you can.
3. If you use any aids or appliances, list them and explain why you use them.
4. If you need physical help, supervision or prompting, whether you actually get it or not, explain what you need and why you need it, including if you need help when using aids or appliances. If you can’t manage even with help from another person and have to have everything done for you, say so.
5. Say when during the day or night you have problems. For example, is it all the time, mostly in the morning or just in the evening?
6. If your condition varies, from day to day or week to week, try to explain how much it varies and what problems you have when you are at your best, worst and average.
