ESA review and submission of evidence

Can I just clarify regarding medical evidence for ESA, does the GP have to emphasise how due to physical disabilities/ difficulties that my functionality to do basic tasks is seriously restricted? So basically we stress what I can't do rather than why I can't ? . Much overlap I feel and surely common sense would tell the assessor this ?
If you can advise me as to key points I need my doctor to state Gordon, I shall be extremely grateful, before I put my letter request to her. Many thanks x

Sorry me again! Can't find anywhere informing me how many points you need to score overall to remain in the support group for ESA.?.
If you don't automatically answer the questions that give the highest points but say you selected all lower ones , can you be awarded the support group status on the total of your score, I've done the self test and scored very highly , however the self test includes the question about would I be at physical harm if I was placed in the fit for wac category, but this question isn't on my ESA50 04/17 form? . Can you enlighten me please . Thanks Gordon x

Ideally you want your GP to confirm that you have problems with the ESA activities and why, at the very least that you have conditions that would reasonably imply the limitations that you have stated on the ESA50. General statements of problems are unlikely to be of value as they will not show that your meet the detailed criteria that ESA has.

Entry to the Support Group is not points based, you must show that you meet one or more SG Descriptors, these are listed in the ESA Claim guides.
If you are trying to qualify through the Substantial Risk criteria then explain this in the additional information section at the back of the form, you will need to show that you meet Regulation 35 to be placed in the SG.

I can't find any info about regulation 35 ?
Is this about being in physical harm if put in fit to work category?
If so , if the GP states this am I covered?

It's in the sections on qualifying for the SG and the one on Substantial Risk. The Mental Health guide has the more detailed explanation.
