ESA/PIPS

I’ve received my ESA50 to fill by 28/4/17 I keep looking
At it and just don’t know where to start. Could I copie my PIP answers onto this form.
I’ve been on support group for 2 years now, I’ve work all my life until 2016 now I’m not fit to work all I seem to do is fill in forms. Because I always worked I can’t get free anything I pay for prescription and eye care at a cost of £400.
I was on the high rate for care/mobility and then pip came in I was placed on standard for both. My assessment was a joke the lady didn’t know what my illness was( P.E pulmonary embolism ). I ask her what her career was she said a Nurse. On the report there was so many lies, it’s a good job my husband attended with me or I would think I was at a different assessment. My GP wrote a detailed health report so I could appeal. I was award mobility- high and care-standard. I’m appealing waiting to go to a tribunal I’m so stressed and can hardly sleep with all the worry they’ve placed on me. Please please help me.
