Hi,
My son was rused a transfer from DLA to PIP. I have written a report to send to the DWP in support of MR. I had written to the DWP saying I would send a CRMR1 form but do I have to include this as well as my own report/evidence?

You don't have to use the MR form, as long as your letter clearly states that you are requesting a Mandatory Reconsideration then it will be OK.
