when pip require medical evidence from your doctor, do they phone your doctor, ask for a letter or send them this form to fill in?

Don't assume that will ask for anything, see

that's why we recommend that claimants source their own evidence.

If they do contact you doctor then it will most likely be by a form, although there is a small possibility that they will phone them.
