Pip 1 form do I need to send in any medical paperw

I'm confused soical worker help me fill in first pip from , on question use my dla medical evidence,I tick yes this from 2013, I got copies from then still. So same medical evidence not change.
I'm under going full care act assessment process now ,the soical worker yesterday, said I don't need to provide medical evidence yet
I'm pip from 2 I do ,she said this report of her full care act assessment report be use for this ,I got 2 week time extension cause of need more time
So is she right,?
I need post get by 16.5.2018 I till all day today ,I sent by special post.
Next on pip I from , question on what other names I'm know by, .
In 4.1.2013 I did send in deed poll , with then my dla claims too.
Do I did need to put my old names they have on my old claims before then in 2010 2012, as dwp dla ESA. Have all Tec information already then to what I known by too,and since 2013 to date information medical NHS official documents is since 2013 too ?

Do you actually have a PIP1 form, this is not usually issued to claimants?

You do not need to send any evidence with the PIP1 form, this should be sent with the PIP2 which you will receive next.

If you changed your name in 2013 then I don't think you need to supply details of your previous names as PIP will only be interested in the previous three years but I would note on the form that you changed your name in 2013.

Yes pip ref on claim from 1, dated 13.5.2018
On wed 16.5.2018 . soical worker in whom assessment in adult soical care act assessment process, call up pip dwp claim number,
Call female handler rushing though information, I nor soical able to understand. Female claim from in post in 2, weeks .it came in 3 days Frist pip ref on from 1007.
Information .it may help with claim to send in, prescription list, GP or hospital letters care , treatment plas , reports of health professionals,
This concern me
.I sent in OT same of 2013 of dla claim, as medical loan equipment not change. The changes in health housing is what I'm local public funded services adult services department carry out detailed care act assessment process still.
So for pip 2 detail from ,she got two week time extension on

Yet she lot more information to require still and hence ,how my joint health illness disability physical illness mental health effects me, only hand wrote too on page my dla claim ESA on pip from what other names your know by,
As only update medical information not resent, mental health, care . Report I'm left no service via referral from medical practice since 2011,
I ended up serious illness since 27.9.2017 why I seek offer care act assessment process since , Gordon is hard to deal with,
As I no family support to help on all dwp. NHS soical services etc to help with pip evidence more so

OK, so your claim appears to have been accepted (PIP1) and you now have a PIP2, How your Disabilities Effect You form (1007).

Or PIP Claim guide explains the criteria you are being asked to meet and how to complete the form

Your OT report from 2013 is a bit old and I think it will carry little weight with those assessing you, have you spoken to your GP about more up to date evidence?
