Are these grounds for a mandatory reconsideration?

The assessors report has many glaring mistakes, most of which the CM didn't pick up on. Examples are:
Writes that we have a wet room with shower then mentions that Carol uses the bath.
A pain killer that has to be signed for as it is so strong - not allowed on repeat prescriptions - is noted as being taken at 8 tablets 4 times a day. Pharmacist says that after max 72 hours she would not be able to function at all.
Accepts that Carol has a very poor grip then states she uses a grabber to pick things up - grip so poor she cannot raise grabber.
Accepts very poor grip but then states she can cook a meal with an aid though assessor was told and on the form we stated that Carol cannot lift anything into an oven and that we cook together.
Says Carol can make toast despite writing that Carol is 100% fat free and hasn't eaten bread for over 20 years.
Failed to mention that I had to support her at end of interview and going to toilet when stated she managed at 30 meter walk unaided. The distance is barely half that.
Due to severe health problems including total deafness Carol had a VERY limited education and cannot read too well and relies on computer with spell check for writing. Told lack of education is not accepted.
She cannot manage household bills or handle money for shopping - marked as can manage complex calculations. Challenged CM and he said such tasks as organizing telephone and energy accounts requires a Financial Adviser???
There are more and one could be excused for thinking she has mixed Carol up with someone else. Carol gets so distressed - cannot mix with groups or deal with "authority" and was physically sick at end of interview.

The only grounds you need for an MR is that you/she feel that she should have received an award or a higher award as applicable.

What I would advise is going back to the PIP Descriptors to make sure that you have shown that she meets them. For example having a wet room will not in itself score points, having a wet room because she cannot get in and out of a standard bath or shower will.


Stick to issues that score points, so taking just what you have written in your post, the issue of the toast will not help her score points.
