I'm afraid you are very unlikely to get a home assessment on your own testimony, you really need your GP to write a letter to the Assessment Provider explain in unambiguous terms why you cannot attend at their offices.
You can use the PIP report but like any evidence you include, you must explain in detail why it is relevant to your ESA claim, although ESA and PIP share some activities, even when they do the criteria can be quite different.

Thank you Gordon, my gp is away at the moment. The secretary has made some adjustment to the original gp letter ie that i have home visits by the gp because i am unable to attend the surgery due to the agoraphobia. Another gp is signing the letter and faxing it on. She isnt able to go into great detail ie how i am affected by it, she can only really add to what the gp has already written. Im waiting for my mental health care worker to return on Friday, she will definately send me them a detailed letter but, not sure if her imput will help. she s not a gp or psychiatrist. As far as i am away it has to be either of the two?

If she is willing to write a letter then I would still send it, it can't hurt (providing it is supportive).

Hi Gorden just a quick update. I can't believe it, i opened a letter this morning from the medical centre. I was shocked, they have cancelled my appointment and told me not to attend. My daughter rang them up and i no longer need to have to have a medical at all. I cant believe it. Maybe they have found the evidence? my GPS also faxed another gp letter of with slightly different wording at the end, instead of saying she didnt feel i could attend for a medical, it stated, she is unable to attend and she feels a home medical was more appropriate, the end of the letter she stated, the gps did home visit. All that stress and anxiety, I really cant believe it. The assessment centre said to my daughter this morning that they have enough evidence to do a paper base? I dont believe just a slight alteration could change there mind, i think my evidence has shown up also. Anyway, its over until the next time. Iv yet to hear about my pip. Thank you so much for your support and putting up with me waffling on, i really was extremely stressed. Now im in shock. Have a lovely bank holiday.Gordon wrote: shazier

Hi /Gordon and thank you. I am still waiting for the dwp letter with there decision. I believe they could still ask for an assessment to be done? My daughter has spoken to the dwp and there hasnt been a decision made as yet by them. I thought it was over but obviously not untill i have received there decision letter. My daughter asked for copy of the report made out by the assessment centre but they don't have it yet? So im not completly sure where i stand at the moment. I thought it was over.
