PIP 'Review Form', not entirely sure I understand!

I have been sent through the forms which say 'its time to review your award'. Though I am having trouble filling it in.
- In the section where is asks how your health condition affects you, if nothing has changed under each descriptor, then do you just leave it blank, say 'nothing as changed', or something else??
- It asks for a 'repeat prescription list' as evidence, what is this and where do I get it from?
- Also can 'care plan' be something from my Psychotherapist? If so what does it need to include?

Also what happens after sending this back, does it involve another assessment?

A repeat prescription list is usually sent out when a prescription is filled, it has a list of all the medication that you can request.

I would expect a Psychotherapist to provide a treatment plan rather than a care one, it's worth you asking for one especially if they have advised your doing any treatment at home.

Once the form is returned an assessor will decide whether a face to face assessment is required or not.
