Dla Appeal Form

Hi there, i hope someone can help me & i do apologise in advance if this has already been covered but i am a new to this. I applied for Dla & was turned down, so i asked for a statement of reasons which i did recieve straight away. At the same time i asked for an appeal form & a copy of any medical reports they had recieved. The thing is i have phoned three weeks running & recieved nothing, each time ive been told you will recieve the appeal form in the next few days and it yet it doesnt arrive. The first time i was told they only had a doctors report, & no hospital reports but last week im told they have do have a hospital report as well. I still havent recieved an appeal form and ive only got another week to send it back so im now running out of time. The statement of reasons contradicted everything i wrote on it, and im beginning to feel they are hoping i will just give up. Has this happened to anyone else or am i just going about things the wrong way? Im sorry for the long post but i would really appreciate any advice please.

Despite having 3 Tribunals cancelled this Year no doubt stalling for time) I now have several large files of paperwork, medical reports etc. I have just come back to square one - I have to submit another GL24 form. I sent the first in May 2009 and this has not yet been re-heard by a Tribunal after the First tier Tribunal was scrapped due to an "error in law" identified by the Upper Tribunal judge. The judge advised my case should be heard at the next possible date, that was Jan 2011) as the panel had not identified properly why my Mobility allowance and care needs had been explained (to-date they have still not answered this question!
So thank you for your post and Gordon for the link for the form. I for one am going round in circles and still determined to keep jumping through the hoops.

The decision-maker's response to my renewal claim form DLA1A was to take away my lower rate personal care component in May 2011. The decision-maker has now decided 11.8.2011 to reinstate this which I am grateful for. I now have two Appeals the new one starts from 11.5.11.

Could I get a more detailed Statement of Reasons on this recent decision.. I have yet to hear the new date of the FtT re-hearing. I hope your Appeal is not as long winded as mine!
