ESA3 Form

Hi -

I looked at the FAQ under ESA Forms, but the ESA3 form does not seem to be listed. This is the form for ESA(C) transfer to ESA(I).

Cannot find it on DWP site or direct.gov.uk also.

Specifically after a PDF version I can download and print as opposed to an interactive version.

Any ideas anyone?

I don't know if the ESA3 form is downloadable, but not all DWP forms are available in PDF; it's mainly the commonly used forms.
