Have to post my ESA50 form tomorrow(mon) as deadline is tues the 18th
But I am still not sure which address to send it to. The pre-paid envelope that came with it is Wolverhampton but the reminder letter chasing the form says t send it to Wembley. Customer services (centre of Health and Disability) at Cardiff said use pre-paid envelope address and ignore the fact that their letter/reminder chasing ESA50 says send it to Wembley. But not sure this Cardiff customer service person knew much (re your previous post disagreeing with what they told me about having to return the ESA50 form ten days before the deadline date). Help please!

Regarding the original pre-paid envelope address of Wolverhampton that came with the ESA50 form , does sending it there enable the form to go onto the DWP system? The reminder letter from the Centre for Health and Disability Assessment that came a week before the deadline , does this say send to the Centre for Health and Disability Assessment Wembley address because they (Maximus) want it sent directly to them because when they send the reminder there is only a week left until the deadline? But if I send it direct to Wembley, will it not get put onto the DWP system? I don't understand - wondering whether I should send a copy of my ESA50 to both Wolverhampton and Wembey???

The ESA50 is returned to Maximus, they will look at it and decide on an assessment and once this process is completed they will return the claimants file to the DWP with the ESA50, any thing attached to it, anything presented at the assessment, any reports that they have collected and the assessment report.

Hi, I have sent my ESA50 off and now am cringing about my answers. I do not know why but I have this annoying tendency to be too positive with all my answers. I really don't know why I do it! One example is I put I try to go out 2 or 3 times a week whereas really I often go out once a week. My psychiatrist has done a letter requesting that I have a paper based assessment or telephone one, he writes that I am almost house bound. Do you think they will take more notice of my too positive statements or more notice of the negative stuff my doctors have written?

Trying to go out is one thing, doing so is different, so which did you write :)

If you said you actually go out this frequently then I would be surprised if you get a home assessment.
