What happened was the original assessor was ill on morning of appointment so new assessor appointed but did not have any of the form information.
Stated that when she went back to office would read my form and collate information then send off to DWP.
She could not have read it because asked for a list of meds, which were on the form.
Also stated that should not bother contacting DWP and requesting report and should wait until I get judgement before requesting.

I don't think that you can assume that asking for a list of meds is proof that they did not read the form, they follow a script for most of the interview and it is likely that this is just one entry on the list.

You can ask for a copy of the report before you get the decision, saying that automatically would get my suspicions aroused as to what is in the report.
Presumably she could not have seen any of your supporting evidence either.
It really puzzles me as to how any of her questions could be relevant without seeing any paperwork. How long did the appointment last?

Had a change of circumstance so was reassessed for pip.
It's been 12 wks now and today after phoning DWP, I was told that my claim needed to be looked at by another decision maker.
I'm a little confused by this, any thoughts appreciated

Hi Steve,
I have merged your query with the other questions relating to your PIP. It helps us enormously if everything relating to one claim is kept in the same place!
I can't say why your claim might be looked at by another DM. What I can say is that PIP decisions to seem to be taking a long time again, having speeded up tremendously at the end of last year. You have 2 choices - to wait until the dreaded envelop drops onto the mat or keep contacting DWP on the assumption that it is the creaky hinge that gets the oil.
In addition, your might want to let your MP know how people are being treated.
