Hi there,I was just wondering if you have have any idea which box i should tick for the questions on washng and dressing ,it gives options for both needing help with upper an lower body now I need help with both so do I tick both boxes of do I tick cannot do it at all ?? Do you have any links for a subject access request for g.p records, I have scoured the internet and can find a template letter sorry if this is not appropriate question .
