I am due my first telephone assessment this week and I scanned and back up copies of what I'd written on the form, because I have memory issues due to having M.S. my problem is that I lost the copies due to my laptop frying on me. can I ask capita or DWP for a copy of the form to be sent out to me.
You can ask them, but I doubt if they will get to you before the assessment.
Having said that, don't worry too much. The assessor will take you through each activity on the form and ask you about how you manage with various activities. If you have the time and energy read through our guides again and make notes about what you want to make sure the assessor hears.
