Hi all is there a template letter on here I could use to request my medical letters and any info from my hospitals I attended over the years for different things please ? Thank you .

What you are looking for is a sample letter for a Subject Access Request (SAR)

The ICO website has such a letter you can use: ico.org.uk/your-data-matters/your-right-...ject-access-request/
