Hi, my grandfather has been getting DLA low rate for getting around and low for personal care since 2008 he has become alot worse past few years with his health and needs a lot more care past several months he phoned up the DLA office and they have send him a form out ( he was born before 1948 so stays on DLA) The first page of the 5 pages pack says you have told us about a change and they put a x in the box Then on page 3 of 5 bottom of it says Important-When you fill in Parts 4 and 5 of this form you only need to tell us WHAT has changed. So does the whole 40 page form need to be completely filled in or just the Parts 4 and 5 to say what has changed ref his personal care/as I do not think they change the getting around part of the claim.
