Applying for a boiler grant based on PIP

My heating boiler has broken and can't be repaired. I am getting PIP and want to apply for an Affordable Warmth grant towards an replacement boiler.

My circumstances are untypical. For example I own a flat which I don't live in because I am too unwell, so I live alone in a friend's house rent free where I have been for several years. I'm not sure if this (and other factors) would disqualify me.

I would like to see a blank copy of the final application for an Affordable Warmth boiler grant but can't find one on Google. Does anyone know where I can download a copy?
