PA4 was the wrong document to request - Please help

I recently had to reapply for PIP.  The DWP did a paper-based review so I did not have a face-to-face or telephone interrogation by Atos etc.  

I received my PIP review decision and have been awarded no points for needing physical help, even though, with my application form, I included a 9 page report from my social worker which repeatedly stated that I need a carer because I need physical support with personal care.

Consequently, I need to obtain the decision maker's report, to see how they justified their selection of (the wrong) descriptors, including the list of medical evidence that was taken into account. 

The document that I need is the PIP equivalent of the ESA85A Medical Report Form.  I was told to request the PA4 but I have received that today and it is the wrong document.  The PA4 is only the report from the Atos "consultant", from 2016 (which I already had).  It is no help to me.  

I only have a few days left to request the correct document, so I really hope that someone can tell me what it is called?

So sorry that whoever you dealt with was so nastily literal. Imho any decent member of staff would have just sent you the recent PA3 with or without the old PA4. This was downright obstructive, make your own mind up what it says about their attitude...

Hi, I had a paper based assessment and there was no PA3 produced as it was done by the decision maker and not by the assessment provider. Just to let you know that there is a chance that a report does not exist.

I hadn't thought of it like that but, you're right, it should have been obvious to the call handler that I needed the most recent report, not one from 2016 that they had already sent me in 2016!  Especially as I began by explaining that I had recently received the decision concerning my review application and therefore needed to see the decision maker's report.  Anyway, I just hope that I can obtain the PA3 in time (the decision letter stated I had to request it within one month of the date on the letter).  It is not surprising (but it is frustrating) that the decision letter does not inform the claimant that the relevant documents, which can be requested, are the PA3 and PA4!  That glaring omission clearly reveals how deliberately obstructive the DWP is!  Anyway, rant over, and thank you for your support!
