Hi Wigster82

I think your best choice is to ring them and they will likely send you out a new form.

When you complete the form you need to put all your conditions and disabilities down with evidence and not just the additional items.

I hope this helps

Regards
Pete

Please Wigster82 consider doing a diary as well, this makes it clear how your more recent conditions affect your daily living/mobility which is what it's all about, not just the labels in the hospital paperwork. Examples and templates in the Guide.

Hi Wigster82 I wouldn't ring them go and ask your advisor for advice wait and let them get in touch with you until then just chill and what ever week you feel like doing a diary etc like someone said above, get any hospital ,clinic, Ask your doctor for a copy of your medical records and medication for your advisor ask or write for support letter and copies scans, MRI's etc of appointments, scan's etc, some doctors won't write a support letter they don't have too mine didn't, some charge which they're shouldn't but you don't have to pay as your medical records and the medication your taking anything else is backup. Contact your mental health therapist etc for a supporting letter, and a supporting letter from a family member or a friend or medical professional who ever see's you the most day to day in side a week. Be cool 82

HI Wigster82,

You have to decide firstly if your health conditions will score you more points on each descriptor and potentially increase your award or not.

If yes: are you prepared to put in for a change of circumstances and gamble on your award being increased which isn't guaranteed?

If no, I wouldn't contact them yet and give a them couple of months at least to send your review form and hopefully the backlog won't be quite as bad as it is at present.

This is just my own opinion, only you can make the decision.

Kind regards,

Phrank.
