Hi Weeoky,
I presume that you are referring to a disability diary for PIP? I am not aware of any template for this, but you don't need one!
Get yourself a blank notebook, or even a few pieces of A4 paper from the printer. The aim of your diary is to note down what happens and if you know, why.
Let me give an example:
Monday 1st
Woke up felt very tired, very unsteady when going to the bathroom
Nearly fell over getting up off the toilet, so sat back down felt dizzy, had to wait 10 minutes before I felt steady enough to get up.
Hands very shaky, unable to get shirt on or to buttons up on shirt. Daughter had to help....
Tuesday 2nd
Needed to post letter, could only get to the church before legs felt weak, and became very breathless. Had to sit down for 10 minutes on the bench to catch my breath. (Wife paced this distance it was 38 metres.)
Etc etc

Note down how you feel, the help you get, and timings. If you feel better say so. It doesn't matter how much or little you write. A relative could help if you are not able. Use the diary to confirm walking distances, stops etc . Distances can be confirmed by a friend etc later. Not only will this provide great evidence to help write your PIP claim/review, it will provide contemporaneous evidence of the time of the claim. Useful if you need to go to a tribunal hearing, which may be several months later, and you might not remember exactly how you felt. (And to compare for any subsequent PIP renewals.) Also, it can help show patterns and variability. So if you can manage to do a diary for a week or two you might notice you are always tired on one day, which seems to link with the previous day/s activities. Or may be whenever you go to the village shop you always feel tired or anxious etc.
Patterns can help you manage your illness, irrespective time of PIP.

Do you mean the diary templates in the PIP area?

Open the diary you are interested in with your browser, you should see the template displayed.

Right-click with your mouse and the menu that is displayed should include options to "save" the template to your computer or to "print" it.
