Is a MR just a template letter?
Or do I give reasons why I want one?
I have read the PA4 form & mode notes - 4 page word doc so far, just picking apart the PA4
Is this what I should use? or wait for a tribunal. I would have years worth of medical records to request for a tribunal. I did already try, but due to covid restrictions I was sent a few CDs that did not work. I asked again for copies - so I need to chase that up.

From the PA4 Lots of information has not been recorded from what I said.
Lots of information are the answers she tried to coerced me into saying or agreeing even when I disagreed.

While the accessor agrees my condition could have a potential impact on being able to do activities. It is of their opinion it is not everyday and only some days. As it is a physical condition, I am struggling to explain how it affects me everyday.
Also the assessor has signed the PA4 as a disability assessor/Nurse. I do not see any nursing qualifications on the assessors linkedIn Profile & cant remember how they introduced themselves. Also my assessment was at 8am & over with before 8.30am, yet the time stamp for all the forms are after 9.10am. Makes we wonder if the rush was because someone had a school run to do!

You may be able to find court cases from pipinfo.net

We do not hear of many cases not taking ages to go through, although now and again they do crop up, it does depend on where you live.

A MR can be just a letter requesting a MR. I usually use a CRMR1 form as I like the format and you can add extra pages, if you do add extra pages make sure you number the pages and add your name and Nation Insurance number on each page. duckduckgo.com/?q=mandatory+reconsideration+form&t=h_&ia=web

I would use the PA4 to pick apart, if you are still turned down use the same document for your appeal

If you are struggling to explain how your condition affects you on a daily basis, try doing a day diary, there is a example in the guide tab.

Quite often we come across forum members who say that the PA4 does not follow what was said at the assessment which is why we advise members to record their assessment.

I would also raise the issue as to when the assessment took place and how long it lasted.

Thanks - unfortunately I did not record the conversation.

Couple of questions if I may :-
Should I take photograph evidence & submit?
It is quite easy to see some of my physical conditions & I do not think the assessor fully understands my rare condition.
My home aids I use at homey was left out by the assessor despite me saying what I have and how I use them. One of the assessors points was that no aids was prescribed! I know an aid does not have to be prescribed to be an aid. I was asked if I have a stairs bannister, I said yes and also said that I have a handrail and grab rail at the top. This was omitted from the PA4

Should I raise the assessors qualifications? It is claimed to be a nurse on the PA4, but this was not told to me at the point of contact & I have further evidence that the person is not a qualified nurse.

The PA4 is time stamped from 9.09am on page 1. My assessment was over with by 8.30. I do not see how this took longer to complete & submit than the telephone assessment! Especially when it is mostly word for word to what the person was trying to get me to agree to on the phone.

My concern is a MR is only done on the information already captured by the assessor. Which is full of missing statements of fact I told the assessor. I am challenging how the assessment was carried out and how the data was not collected. The assessor role should be to ask questions & let me answer. Not ask questions and try to give me a range of answers to choose from.

The Muscular skeletal system section only has the wording Assessment undertaken by telephone.
The assessor would already be aware I am waiting to see an MSK specialist as that is where I have been referred to.

I really need the assessment completed again by someone competent and qualified.

I would follow the instructions I gave you in my first post.

The first thing you need to do is go through the PA4 with a highlighter highlighting all the inaccuracies and making a note of any omissions.

Then complete a MR form or letter whichever you prefer to use quoting the inaccuracies and omissions. I usually work down the PA4 starting with check list of evidence considered, then History.

In your case you may want to start at the first page, Time consultation started ... Time consultation finished.

There is no one way to complete a MR, it is up to each individual, everyone has their own way of doing things.

I have already gone through the whole document.

Really annoyed how the assessor tried to spoon feed me answers and none of my objections to the spoon feeding actually translates into what I said.

Every question looks like a cookie cutter / pre determined answer before even talking to me!
Banging my head now that I did not record the call. It is not as if I dont have the technology either.

I will probably add some photos then.
