Would it be possible to provide a template for Pip 2 review letter that would enable me to type in the information to keep as a record for what I supply to the DWP. They have sent me a written form which I will have to get someone else to complete this for me due to my tremors in my right hand.
I'm sorry, but that's not something that we have. I'm afraid whoever is going to do it for you - will have to complete on separate sheets of paper. They don't have to type out all the questions - just make it clear which question is being answered by the numbers on the form.
