You can find information on how to complete the review form - page 129 onwards in the Guide to PIP claims and reviews benefitsandwork.co.uk/guides-for-claimants/pip

You can write a note requesting a paper-based assessment - though you will need to explain why. Only a small percentage of paper-based assessments are carried out (less than 10%).
