Requesting a Copy of the PA4 Assessment Report was created by beakie
Hiya guys
This week I have rang the DWP twice requesting a copy of my assessment from Monday 6th June. On Wednesday 4pm(48hrs) I was told I needed to wait 72Hrs.
I have rung this morning requesting a copy and was told it was still not available and that sometimes they need to be audited and sometimes altered(doesnt fill me with confidence if they can do that). I requested that I spoke to someone more senior and got transferred to a Case Manager, who contradicted herself by saying, I was entitled to a copy of the PA4, and then said only after a Decision Maker had looked at the report, which I then said defeated the point of requesting the report in the first place. I then said I understood the point of requesting a copy so soon, was so I could write and make a case for anything I disagreed with in the report. She then said that wasn't something I could. I told her I had a copy of a previous PA4 from Feb 2020, she replied "well I don't know how you got that".
Now I'm worried I stired a hornet's nest and it might affect any future decision.
I'm sorry...... but I think i'm venting here, a little. I'm so frustrated, after hold's of 45mins in both cases
Just wandering if other people are geting similiar experiences. I'm stumped

Hi beakie
The case manager is telling you a load of rubbish. They do like to try it on. You are entitled to a copy of the PA4 after 48 hrs. You could go back to the assessment services and ask them why it is not there and then you will find out whether your form is being audited. If it isn't being audited, then you should question why it isn't there.
After that I would keep ringing the DWP. You will find different call handlers giving you different answers which is sad but true. You are entitled to that report before the decision and there are plenty of members here who have had theirs.
You have not opened up a hornet's nest and it will not have any impact on the decision.

Hi Bis
Many thx for your reply.....I will contact Capita, thats a good point.....
I had thought about writing to the DWP to request a copy of the PA4, thinking they would have to respond more to a letter, but I'm not certain about it.
Many thanks again the to the forum.

Dear David, do have confidence in BIS's reply. You were totally right in what you told the biased and untruthful case manager.
I did it after discovering this here, and writing that letter I am certain saved our daughter's 3rd claim/renewal from having to go all the way to Tribunal as the first two had. So you can imagine how glad I am that I did write. Luckily they texted our daughter to say the PA3 [as it was a paper-based assessment] had been submitted to DWP, so I knew to call for it and did get it sent promptly. The usual advice [for other readers] is that if you call for the report and are refused, just politely end the call and try again.
If the report has untruths, you can coldly debunk them by referring to your original evidence and if necessary additions such as NHS pages on what a medication is for [eg Tramadol is for severe pain, not moderate as the assessor had tried to say]. Just don't use the word 'lie'.
Best wishes,
Denby

Hi Guys....just a quick update.
Rang Capita to day and my report isn't availble after week today. The nice lady said it was being made more robust? I said I would ring in a couple of days and I was advised to leave it till the the end of the week.
DWP last week said they do audits on the assessment reports, which I dont like the sound off.
Many thx all.
Best regards
David
