Requesting a fit note when I’m still on maternity leave

Hi,

I am still officialy signed onto my work, after being on maternity leave, and I’m due to start work on 29/07/2022. However, my maternity pay has ran out and I will not have any more pay on those last two mo this before I’m meant to start work.

I know I won’t be able to go back to work as my mental health has severely deteriorated. When I applied for ESA online it asked me if I was employed, so I said yes, then it asked if I was off work, so I said yes, and then after submitting the form it says I’ll need a sick note, but I have not needed a sick note, I only have my MATB1 form from pregnancy, but the notes said that was only if I’m pregnant. I thought the only evidence I would need would be prescriptions, doctor checking etc, and I was to fill out the large form to be sent to me? So I’m worried I’ve filled the online form out incorrectly already.

I could only apply for New Style ESA it seems, I’m not sure if I’ve applied to the right one though? Have I gone wrong somewhere? I’m so confused and worried I’ve messed up everything.

You have not gone wrong, New Style ESA is a Contribution Based benefit and is not means tested with the exception of pension income which can reduce the amount that you receive.

The Income Related benefit that goes with NS ESA is Universal Credit.

In your post you state that you 'I won’t be able to go back to work as my mental health has severely deteriorated' .

The large form that you have been sent is called a UC50 form or Work Capability Assessment. We do have guides to help you complete this form; help-for-claimants/esa1
