PIP Review Form was created by Kieran
How much detail is needed? As the pages are designed to be limited on space, unlike the application forms. If it’s of relevance, I have a strong supporting letter to add to evidence and of course my existing evidence, from the same organisation that provided me a letter last time on my claim.

Hi Kieran
You can add as much detail as you want if it will help with your claim.
It is perfectly OK for you to type your answers and attach them as extra pages to the form, just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.
I would not split your answers across the form and attached pages, do one or the other for each activity (not question), splitting it risks the second part of your answer not being read.
Attach the pages to the back of the form using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

I am so glad I popped onto the forum this morning and read this. I’ve typed up all the answers for my husband’s PIP review and was planning on filling in the form as much as space would allow and continuing the answers separately. It never occurred to me that it wouldn’t get read.
Thanks so much for the heads up Gary.

So where it says Health condition, I don’t have room to write everything down as there are over 20 different ones, do I answer nothing on the form and just say see attached page one?
And for medication can I just put in my prescription copy or do I write them all down?
I got my MS from the Dr and I have highlighted the important bits but it’s 4 pages for the first year and a half. And I don’t want to send too much in, in case they think it’s too much and don’t read it. How much should I send? Thanks

Hi D Devlin
I would write down all your health conditions in date order, along with any medication.

Hi D Devlin, just stating the obvious, in case. Don't forget to photocopy or at least photograph carefully on your phone, every page. This will enable you to avoid getting tripped up by giving different answers if you are put through an assessment as most are.
I photocopied the return address so it could show through the envelope window, as the one on the back of the booklet was of course hidden by all my additional sheets duly treasury tagged onto the form in strict number order.
And don't forget to send it using tracked post. Being short of time I've posted one recently. On a tip from here I used the envelope supplied which is prepaid for second class post. I paid the top up so even for special delivery guaranteed before 1pm next day, it was £7.65 minus £2.15. So I only had to pay £5.50.
Peace of mind is worth it. I always check the tracking and print off the signature when it has been delivered.
Oh and by the way, if given an assessment appointment, immediately inform them you would like it recorded. Best not take chances.
Best wishes,
Denby
