An assessors report (PA4) is produced when you have either a telephone/face to face assessment.

From the assessors report the Decision maker will make their decision, if you ask for an Mandatory Reconsideration, a different decision maker will look at any new supporting documents, PA4 and original supporting documents, latest stats show that only about 20% of MR claims are successful, those claims that go on to the appeal stage, about 70 % are successful.
