A lot of you may know this already but I wonder if this information might help some of your users?

I found I was hitting my head against a brick wall trying to obtain medical evidence, even when offering to pay.

In one instance, I wanted evidence of my physio assessment, treatment and outcome. The department were too busy/too much time had elapsed etc etc.

I finally found a solution and I had the documents and details I needed, hand delivered to my home, free of charge, within 2 weeks.

Each healthcare trust has a form you can fill out and return via post or email. It is a simple, short form which you send off with your ID (In my case a photo of my passport).

My healthcare trust is Northumbria Healthcare so I googled:

Northumbria Healthcare access to records

The form was the second search result that popped up.

Out of interest, I have tried googling the same but using different healthcare trusts and the results are the same - you get access to the individual healthcare trust's Medical Records Request Form.

I really couldn't believe how simple and easy it was!

Hi Gary, I'm pleased you think it might be useful. Here is a link to the Northumbria Healthcare form to give people an example of how simple it is.
