Redacted PIP assessment document identification was created by Tony
A Subject Access Request yielded a two page document, at the end of the telephone PIP Assessment, the contents of which were redacted.
I just wondered what this document might have been, or is called?

Hi Tony,
Who knows!
Is any part of the document visible, eg a form number, part of the words?
I suggest you write back to to SAR department and request n explanation of what has been redacted, and why. You may not get an answer but given this is SAR request there should at least be an explanation. Eg assessor's home phone number scribbled on form, or whatever.
I haven't had this situation before. Maybe other mods or members can advise further?

Sorry I thought I had replied to this. No, the title was redacted. A block of text, pretty much half the page was redacted. Because it was redacted I would assume it was a risk assessment over the mental health element of the claimants claim. Or least touches on her mental health. The image is uploaded below to a random image host, with the claimants details 'digitally' Tipp-Ex'd out.

It could be something else but the DWP and its agents are required by law, unless authorised by a relevant doctor, to withhold what is known as Harmful Information from a claimant who is regarded as at risk.

Yes, that's my thinking; though 'why' it was redacted is of less interest than 'what' this document is.
As said it was present in the SAR, but completely absent from the document bundle for the tribunal.
But it clearly is associated with the PIP assessment, was written by the assessor, so appears to be part of it.
