Hi, my husband was my full time carer, he did everything for me and last Jan 2021 he died unexpectedly.
He completed all my PIP and esa claims with obviously my input. I've got copies of all passed pip forms and I have been successful with being accepted.
My conditions have got worse, and my mother has had to move in with me to help me as much as she can. Everything that has been on my forms throughout are still the same, but there are additional issues which I will need to add. However I'm unsure what I should add for these 12 questions, that havnt already been included in past forms. Do I write the same points, and add the additional issues. I'm finding this whole process extremely daunting and I would be grateful for your advice as to how I should complete the form

Welcome to the forum. Sorry to hear about your loss.

You might want to have a look at the following FAQ which explains where everything is; www.benefitsandwork.co.uk/guides-for-claimants/faq/forum

If you reported a change then you would have received a PIP2 to complete this was the form that you filled in when you originally applied for PIP. If you have a copy of your previous claims form then use that as a guide and add any changes in your condition, remember you have to have a condition for 3 months and you will need to show that the deterioration of your health is likely to continue for the next 9 months.

Before you report a Change you should be certain that you would reasonably qualify for a higher award, not all deterioration or even new conditions will result in your scoring the points you need for a higher award. I wonder if your PIP award is moving towards its end, and so the DWP has sent you a PIP Award Review form, the AR1 form?

If so, then I found that pages 128 to 134 of the benefits-and-work Guide to PIP claims and reviews is really helpful. These page numbers apply to the latest version of the Guide - this is version 56, September 2022.

Thank you for your kind words, it's hard enough as it is losing a loved one, he was my strength on the very bad days and now he's gone it's very difficult just to be motivated getting up in the morning. Having to deal with this form is so stressful, and really makes me so anxious.

Hi, thank you for your kind words, and also for the guidance in dealing with this pip process. I have help from my mum and my next door neighbour, hoping between the 3 of us we'll be successful
