It is unclear which form you are asking about, if a PIP2 then there is a sample form on the following link

If an AR1 Review form then the best I can come up with is the following

No, there isn't a form you can fill in without the specimen quote.

Thank you for your offer of the review form,

I believe the review form may have a QR code printed on each page and therefore would not help people like CW as they could not have used it as an original form, although they may have been able to type on the form, print off the completed form, then cut out the answers and stick them onto the original form, especially if they have hand writing like mine, which I frequently cannot read.
