Pip PA4 list all evidence alongside the consultation findings /other was created by L20100
Hi, due to go a review shortly via call, tommorow. I have managed to look out all paperwork also having a prep day, but seen on completed form consultation report form pa4 I had done in 2018 after a facetoface, that on page one under the heading of list all evidence considered alongside the consultation findings it is just listed as pip questionnaire, and the section below with other (please list)
Says all other evidence considered.
I'm wondering if this is ment to list all medical files/evidence as looking at the way it's displayed it was just the consultation on the day points that was used. Unsure if someone can say if this indeed to be listed appropriately reports eg orthopedics etc, as you submit with your initial claim form. As surley if a dession maker just seen non listed reports they'd not have a accurate, decsion to go on other than the assessors report on the day.
I was just wondering if this indeed wrong, or if folks can confirm if this has to be listed to be legible eg for correct assessment or just their way of looking like they've your evidence but just not listed it, and if this can be challenged.

PA4 very rarely lists all the evidence that people have sent in, and some claimants successfully argue that key evidence they sent in was ignored when writing the assessment report and when the decision is made. The assessor usually highlights the evidence that has carried weight with them.

Hi, thank you for letting me know, this as important reports you gather after surgery and in my case many years later to same devision, with further xrays & degenerative prognosis and then terminal restriction degeneration on movements listed both upper limbs in this report, should be considered, as one of the main ones I would have thought they could list in that the five lower boxes above other before went to decision. This is quite alarming to see, as had previous esa paper based gp which does detail decisions used as evidence more detailed, along with consultation of the Dbc, which as a claiment is somewhat reassuring that its been explored in detail better.
My call yesterday lasted 2 hours... and had thought listed questions on change would be main on review, but hoping after asking for a recorded session and saying about evidence used and section to list part, hopefully gets completed and listed as such or this be two now without correct boxes filled, before dm decides. Received a text msg today that they have report,. I'm awaiting request also for Esa access as some of descriptors, though similar do relate & can also be filled.
