Page 11 PIP Application form: name of treatment, therapy or operation

Hi,
2 quick questions:

I have severe depression and anxiety.

1. Does an 'assessment' for mental health actually come under 'therapy or treatment'? (on page 11) - it's a report for my GP.

2. I have a letter from CBT therapist detailing my therapy with her - this indicates very good PHQ-9 and GAD scores at the end of therapy- which were only temporary and have now reverted back to severe. Should I use this form of evidence? or explain why the PHQ-9 and GAD scores have changed?

Only you can decide what you want to include as your evidence, but we always advise claimants not to include any information that could hurt their claim.

A member of B&W recently submitted evidence from her GP that said her "condition had improved." Well, it had in that she could get out of bed when she hadn't been able to before, but the DWP took it that she no longer had any problems. It is always difficult to explain something like that when it comes from a medical professional, even if the interpretation is wrong. Claimants are not allowed to lie about their conditions, but it is their choice over what evidence they choose to submit.

You can put the assessment report under therapy or treatment. Always make sure that you have some narrative about it related to the PIP criteria - not just include it, but don't mention it. Eg when talking about going out which must be challenging with severe depression you want to describe your difficulties and then say "see report X for GP" - so the reader makes the link.
