UC ESA 113 was created by Shaisum

Can I request a copy of the 113 form xompleted by my GP? Do I ask the DWP for this or my GP?

Hi Shaisum

You can ask your GP, but in my experience, most GPs don't keep copies of the forms they send to the DWP. So it will be the DWP.
