hi there
i have some questions about doing a mandatory reconsideration- ive tried my best to read guide but ironically because of my fuzzy head /mental health issues- its is difficult to take it all in. last time i did my MR myself- and WON! BUT I just wrote it-covering all the relevant points- as assessor had lied about what happened/ what was said in room - ( i had my sis with me so she wrote a supporting letter) & assessor put in decision that as i could answer all questions- make eye contact- seemed intelligent! etc 'my mental health did not affect my daily living' -which was ridiculous! so i could challenge it - & won -BUT- i had had a face to face assessment!! & the old gp report was filled out correctly. as previously stated-they cancelled my tel assessment so i didnt get a chance to explain further my conditions and daily living needs , MENTAL HEALTH- DPERESSION AFFECTS ME & meds side affects- constantly fuzzy headed & get dizzy with panic attacks help /aids needed etc as they did a 'paper based 'so i didnt get a chance to fully explain things in more detail. as paper based - so no imput form me directly- they only hv my form - an old mental health team report/ support letter - (it took 2 years to sned fom to atos) & GP report form -which as I explained before - because i wasnt expecting GP to call- the receptionist had told me he was fully booked & would NOT BE CALLNG THAT DAY AND i stupidly didnt think to look up gp report form- as i had copy of my old one that just had conditions & meds so i didnt know the form had changed! so when he called unexpectedly -& suddenly started asking what help i need - i was in such a state i forgot to tell him important info- eg i use a pill Dossett box to manage my meds etc AND he asked what do i spend my money on!!! wtf? totally unexpected and i went blank & didnt tell him things i use money for to help my mental health etc - im sure i will b turned down. ironically if i wasnt so genuinely unwell this would not hv happened. & iys affected my health terribly. everything seems more complicated than when i first claimed! questions are_ do i have to fill out a form - or can i just write my reasons in each relevant point form- or do i hv to fill out form. can i send an older mental health report - i didnt send with this review that explains my ptsd cannot be cured? . or will they say thats old evidence even to its relevant. Can i explain i wasnt expecting hp call so didn't say certain things that affect me /help i need etc . can i use the 50% rule - as gp knows i cant go out more than 2 days max. and would hv written in conditions 'cant go out ' can is send letter from family member who has come to stay several times since last pip application and witnessed my conditions , self neglect- not washing bathing - not dressing inability to function properly my incontinence etc etc - or will they discount a letter from family /say family can 'collude'? if i have no new medical evidence APART FROM LATEST MEDICATION LIST- will they automatically fail me?- i dont want to call the local metal health crisis team again - even tho i constantly hv suicidal thoughts because ALL of my previous contact with them has not been helpful- its just been upsetting & incredibly damaging. Hxxxxxxx mental health team hv a terrible reputation - they r so underfunded under staffed- unorganised- many staff are so under trained- overworked -rude - damaging -its literally a nightmare! ( ong story shared by other users) im afraid of them. so i dont know how i can get new evidence . any advice knowledge appreciated. i intend to speak to gp again- but hes not there again for 4 weeks!!!! this whole thing has taken over my life - & many days i can barely function at all-keep having to go back to bed- am so scared all the time my ptsd has really kicked off! and hv no help or support so im trying to struggle thro this best i can. so sorry if i keep asking questions.

I have read your post a few times but have to admit I am struggling as you mention a paper assessment then go on to say do you need to fill out a form.
You state that last time you managed to do a MR yourself and won ... it would not have been an easy task so well done and give yourself a pat on the back. I assume that you have been sent a review form.
If you receive help from family members then ask them to write you a supporting letter as to what help they provide and why they provide the help. The why part is the important part.
If you have had a decision letter based on a paper assessment then ask for a copy of the assessment called a PA3.
If there has been no change in your condition from your last MR then send in a copy of the last decision and inform that your condition has not changed or that it has deteriorated but then explain what has deteriorated. If you do have a review form to complete we usually advise claimants not just to write no change but explain how their condition affects them but for some claimants this will be a struggle, especially if they have no help. You do need to get the form back to the DWP within the time limit and send it by registered post.
Do you have a family member who can help you or a local Welfare Rights Organisation?

HI ther to explain further when i did the ORIGIONAL PIP APPLICATION i had f2f assessment my sis came with me so when assessor lied she wrote supporting letter stating what actually happened in f2f assessment - AND I WROTE MY OWN MR via a kind of essay with each descriptor bullet points & how ridiculous the asessors conclusions were and won.
this is about my first and CURRANT PIP REVIEW!! ATOS had given me a telephone assessment date march 13h then out of blue- my tel assessment was CANCELLED ! AND THEY SUDDENLUY DID PAPER BASED so i was UNABLE not given the chance to speak with them to explain more fully how my health affects my daily living needs as as i was with the mental health team at time of filling out pip review form and they were stressing me out so much - i didnt do a good job of filling in the pip review form well. AND Atos did paper based BEFORE GETING MY GP REPORT. because of my fuzzy head- i didnt think to look up the NEW GP REPORT it has changed SIGNIFICANTLY since last time it was done!- last time it was just medical conditions and medications. so with new gp report - i had called at 8am when i cant think straight coz im nocturnal & the receptionist toldme my GP was fully booked so would NOT be calling me! IF she had said i had a tel gp consult booked- i was going to write notes & would hv looked up new gp report in pip guide-so i was ready - but as she said he was NOT going to call- i was going to write notes later - when i had woken up - as im nocturnal i cant think in mornings - then he called totolly out of the blue when i wasn't expecting it - he started asking questions i was NOT expecting - eg 'what help do you need' & i TOTALLY FORGOT IMPORTANT THINGS - EG HELP GOING OUT - help FOLLOWING UNFAMILIAR JOURNEY - monitor health & meds i use pill dosset box etc. AND he asked what do i spend my money on & i just froze! this would NOT HV HAPPEND if id known GP WAS GOING TO CALL! so upshot is im sure i will FAIL /LOSE MY PIP cant pay bills without it etc.

Follow my instructions in my last post, as you have had a paper assessment then you need to contact the DWP and ask for a copy of the assessors report called a PA3. Thee are occasions when you may be told that no PA3 report was filed.
If a PA3 report was filed and DWP sends you a copy then go through the report with a highlighter highlighting any errors or inaccuracies. Once this is all done, go on line and download a form called CRMR1; www.gov.uk/government/publications/chall...ork-and-pensions-dwp and complete the form explaining all the inaccuracies in the PA3 document, you can also send in any supporting documents you have.

firstly you say a PA3 assessors report is what i need to ask for but sometimes they say a PA3 may not be filed. if they say a PA3 report had not been filed what does that mean? can they a\ctually make a decision without a PA3 report being filed? or does it just mean it has not been filed YET?
Also is it worth including a letter from my mum as she has witnessed my self neglect , and health issues when she has visited she visits for 3 days at a a time so has witnessed my health over 3 days - will they take that into account or just ignore it because its a family member?
Also can i call to say i want to ask for an MR so its on record but that i will put in writing either by the form or in writing my reasons? thats what i did last time. i called to say i want MR then wrote my reasons.
i still hv not recieved a decision letter tho im sure it will be No. im so unwell at mo im waiting for actually getting the letter before i call and ask for report if they don't provide it. and ask for MR .

Also CAN I CALL DWP to say i want an MR but i will put it in writing as i cant think of everything on the phone and will need some time to write my argument/reasons i disagree with decision due to my bad health. and how long do you have to get the MR form or info in writing form the time you ask for an MR?
