Hello,
Is it possible to that you don’t have to fill in the UC50 form for LCWRA? The reason why I ask is because my work coach has asked me not to fill it in. Her reasons is because of my psychiatrist letters and she said that they are sufficient medical evidence.
I understand that they are the ones who put in the referral to the work assessment company, but she said my referral is different so I don’t have to fill in the form.
Any clarity or experiences with this would be very much appreciated.
