I posted earlier today to ask if anyone has completed the UC form. My brother suffers from mental health and I am his appointee . I will be helping him with the form and I am worried sick if I mess up for him. I just asking if the form is straight forward and able to understand the questions or is it difficult making it more challenging. I thank anyone who can reply.
