Disclosing help for the form was created by Kaitlin399
Hi!
I am working through the UC Guidance. I am Appointee for my daughter. The information is so helpful. I feel like I am with a knowledgeable person who is giving me personal advice. It's a hard slog going through it but so worth it.
I had also phoned a Welfare Rights place to check a few things and they gave me inefficient information which if I had followed would have been detrimental to this application, especially as I am seeking substantial risk to be applied.

My question is do we disclose on the form any help with completing it?
Would I put Benefits and Work?

Thank you.

Hi Kaitlin399,

As far as I remember the UC50 asks if someone has actually filled in the form for you.
Is this the case with your form ?

Hi Kaitlin399,
