Hello,
I hope someone, can help.
I'm completing the medical form and so far have identified points 7 and 8 as issues that I have difficulty with.
I've read the guide but am having difficulty understanding what p 7 & 8 are asking.
On point 7 I can hear what someone else is saying - I've no issues with my hearing but do have issues with sight loss and can't read large print and don't know braille.
- I can't see hazards, such as, fire signs I use audio books, speech talking software on phone and computer. In shops and cafes I've to ask for assistance with prices and location of items and assistance with bus timetables and bus numbers. Do I put these issues on the form?

Point 8 - I use a long white cane and any new routes (unfamiliar places) are planned in advance with a rehab worker. But, I can't navigate safely if there's an obstacle on the road, kerbs or bollards. I'm confused by what the form is asking really and need a bit of guidance. I usually just use my cane for routes without someone coming with me. If I'm going somewhere new a route is planned in advance with a rehab worker.If it is to become a regular route. If I'm going somewhere such as a hospital appointment which usually is a one off occurrence then someone would accompany me in that instance. Do I put this information on the form?

Any guidance would be appreciated, I'm very confused.

Activities 1 to 10 cover physical disabilities and must arise from a physical condition. Disability Rights Handbook Edition 49

Activity 7 Communicating - hearing and reading

This activity is relevant if you have hearing or visual problems. you are asked two questions.
first, whether or not you can unstained simple messages (such as the location of a fire escape) from other people by hearing or lip reading without the help of another person.

second, whether or not you can understand simple messages from other people by reading large-size print or using Braille.

Activity 8 Getting around safely

This activity focuses on your ability to navigate and get around safely. It may apply if you have visual problems.
if your vision has only recently deteriorated, this may have affected your confidence and should be taken into account. if your vision varies from day to day, describe what you are like on both bad days and good days and how frequently the former occur.

Give details of any situations that have occurred when you have tried to get around without someone with you, for example bumping into things for people, getting lost or incidents with traffic. Write down if your driving licence has been withdrawn because of your visual problems.
