UC Application Form

Hi, I seem to remember B and W did an online UC claim form a while back Not an official one of course but a sample so members could do a dry run of the initial application. I can't find it anywhere on B and W Resources Could a Moderator provide a link for me if you can find it Much appreciated

silly question David this is a dummy form isn't it? Like a dry run. I'm not actually applying to DWP by filling this in?
