Hello, hope you are well.
Could you please help me with questions 17, 18 and 19. Nothing has changed but I was advised to write something in the box that points tona copy of the original form that I will be sending along with the Review form.

Hi, sorry I didn't see the reply to the previous forum post. Why are you advising against this? It would be a copy of the form. Plus nothing has changed there isn't anything to add. Due my Disabilities I can't write much at all and unfortunately the Agency providing my Support Workers went out of Business a few weeks ago and the local Council still hasn't sorted out new Support for me. Thank you

Hello Gary, hope you are well.
I meant that I will be attaching a copy of the original PIP form. It won't be the original. So aa nothing has changed since that original application and I struggle with pain when I write. This is my work around.

There is no need to send a copy of your original form - they already have it on file, but if you want to send them a copy - that's up to you. You say that writing is painful - so I assume typing is too - but you would still be better to put in a couple of sentences rather than nothing. On your other post you asked how to answer the questions below: I can't tell you exactly how to answer them because I know nothing about you. But, I have given a short answer where you could complete the sentence filling in the information in the brackets.
17 Have there been any changes in your health condition or disability since we last
awarded you PIP? There have been no changes in my health since (date). I continue to suffer from (your disabilities)
18 Have your daily living needs changed since we last made a decision on your PIP? By daily living we mean preparing food and cooking, eating and drinking, managing treatments and taking medication, washing and bathing, managing toilet needs, dressing and undressing, communicating, reading, mixing with other people and managing money.
There have been no changes to my needs since (date). I continue to struggle with (your words) because of the pain I experience from my (your disability), and I need support with (your words)
19 Have your mobility needs changed since we last made a decision on your PIP? By mobility we mean getting around and planning and following a journey. My mobility needs are that same since (date). I still struggle with my mobility because of the pain I am in due to my (your disabilites/s) and because of the pain, I struggle to concentrate that makes planning journeys (difficult or impossible).
