hi im woundering if any one can help me,i have a couple of questions recarding the adult dla form.
I having been trying to get dla for the past 5 years based on the fact i suffer from anxiety/panic dissorder/depression and insomnia but have not been sucessfull once
my questions are
1. what would be an apropraite answer to put in the box based on stumbles from panic attacks and side effects from medication were it asks How many times did you fall last month/year.i mean unless you actually keep a diary this is a pretty stupid question,but i dont want to put to little in or exagerate
2. since i suffer from aniety/depression/insomnia,when things get to much for me i sometimes self medicate with alcohol mixed with non prescribed drugs(diazapam) which have in the past led me to passing out and collapsing,should i mention this or will it go against me if they know i sometimes self medicate
All help would be most welcome,thanx

I'm afraid it's impossible to give advice on individual claims, as is explained in the Frequently Asked Questions (FAQ)

You need to seek 'face to face' advice from an agency such as your local CAB, or contact Community Legal Advice
