must your name and nino be on an EMP report urgent

hi every one i need help and 100/% clarication , There is no name or NINO on the EMP report i received when i requested all files for my appeal for DLA HRC+HRM indefinate award for the past ,8 years, I have contacted medical services and DLA agency regarding this and am getting nowhere ,Both have written back and have still not clarified my question ,both saying practicaly the same thing ,QUOTE FROM REPLIES (As the report form which the doctor completes comes in booklet form name and national insurance number is not on every page )

But they have still not confirmed it should be at least on the front page ,even though i ask them that question ,when i queried how did they know or i know the report was mine ,
As i want to submit this fact at my appeal i need to be completly accurate ,

all help greatly appreciated , letitia

Your name and NI number should certainly be on the first page of the EMP report.

In looking at a specimen EMP report, those details should in fact have been recorded by the DLA Unit, before the form was passed to the EMP. If there is no name/NI number recorded anywhere on the report there is, as you correctly say, no certainty that it refers to the DLA claimant?

In my view, it is arguable before an appeal tribunal that the report should be completely disregarded, in the absence of any reference to the claimant/appellant.

many thanks CD,

your an angel in disquise ,the EMP report is so full of inacurries as well ,and one statement she wrote that i had put on a bra and fastened it and in brackets she had written (demonstrated ),which i wrote to medical services about along with other things she had written that were untrue , and i have now received in writing from medical services ,that the EMP at the investigation on the complaint i made regarding her report ,The EMP has now said (I didnt actually put a bra on but in her opinion i could have as i reached behind me to an appropiate level )
Which is another untruth as i didnt reach behind me at all ,
