consultants reports

hi wondered if any one could help.i have applied again for dla (been turned down twice over 2 years) but my health has deteriated badly now have major mobility problems.anyway to cut a long story short was advised by my occupational therapist to obtain copies of any letters sent out about me by any consultant or hospital appointment but forgot to ask her how to go about this (got a memory like a sieve lol)and now have had a letter from dwp saying they are to write to my consultant dont know which one as i see a few, but wondered if anyone on here could advise what i had to do to obtain copies of letters etc and any idea how much it would cost

You will need to make an application in accordance with the Access to Medical Records Act 1990.
