I have just about managed to print off this guide,took me ages to work out printer though! I really wished I had looked at this guide before I completed the ATOS form.My Medical is 12th August
Can I fill the questionnaire in and take it with me to the medical as extra evidence? Also have managed to ring my GP up this evening. I just really want to know before hand, if he can tell me what he has written.The receptionist told me to ring back tomorrow morning and she would ask a GP to talk to me. I am trying to get a picture of what supporting evidence there may well be. I am also going to speak to my social worker and CPN (no disrespect to the proffesion,but she has been useless) I have been left for weeks in the house (I have 3 children) and have had to rely on their father to take my youngest to school,go shopping and do everything..And he has had to go to work!! I made a complaint to the social services about her,when I couldn't fetch my youngest from school last week.She came round,not looking too pleased and promised lots AGAIN..Haven't heard anything.I always ring 1471 because sometimes I get quite anxious when I don't know who is ringing, and her number hasn't come up..How the hell am I going to win an appeal,if this is the case,if I have people like her supposedly looking after me. Some people who have posted on here seem to have representation,which is brill..Is it just me? I feel I have to fight this on my own and it's really upsetting.

I hope you meant WCA not PCA
I'm not sure what questionnaire you are referring to, but it is up to you to decide what evidence you use.
When people talk about Representation it is usually in the context of an Appeal, however, you are entitled to take someone with you to the medical and we would encourage you to do so, if only to have someone there to support you on the day.

You can send further information to Medical Services and take a copy with you to the face-to-face assessment in case they haven't received it. It's not necessary and probably not advisable to write it on another ESA50, which would not be your official questionnaire and may cause some confusion at tribunal stage (not that I'm suggesting it will go that far ).

I am not very clued up about it at all,and I think I mean the work capability assessment form. The form I had to fill in was the one where you have to tick boxes and 'try' to explain about your illness,for ATOS. It's all very confusing for me. I have already began to put things in place,I have written to my MP and also contacted the law center,about representation,if I need it. I feel alone in all this,but I know I have to be ready.
