Can I just photocopy the original that I have?
Also, if I receive the ESA65 etc in time can I attach them as 'evidence'?

Does anyone know where I can print off a DLA claim form. I have an orginal which says at the bottom 'DLA1 Adult February 2011 (Revised)', and would like to print a copy in case of mistakes, but the claim form on the direct gov website and says 'DLA1A Adult February 2011' , so I presume there is a slight difference.
Can I just photocopy the original that I have?
Also, if I receive the ESA65 etc in time can I attach them as 'evidence'?

If you have been sent a DLA form I would suggest you complete it and then photocopy it. If you feel the ESA65 is helpful to your claim for DLA by all means attach it. They are separate benefits however.
I know the DLA form is repetetive and tiresome but it is essential to complete all sections properly and not rely on the ESA document.

What I was asking is can I take a copy before (not after) I complete it so that I have a spare in case I make mistakes. I will of course also take a copy when I complete it. I understand that ESA and DLA are separate.

download this and save to desktop then fill in as you have time bit by bit then print off as many copies you want

Yes you can, but I dont think you are not taking the right approach, its up to you of course. As you will spend sometime completing the DLA form its recommended to have a photocopy of what you said.
When you return the DLA form to Blackpool that's what the Decision Maker will base his/her decision on. Having a blank copy and relying on your memory if its required to me does not seem sensible.
If you're assuming you will need to submit further evidence at some point, you do not need to send another DLA form and you will have to start all over again!
